Alzheimer's disease (AD) is a neurodegenerative disorder characterized by abnormal accumulation of tau protein, which plays a crucial role in maintaining the structure of nerve cells. Recent findings suggest that the phosphorylated tau fragments, such as p-tau181, p-tau217, p-tau231, and p-tau202 and 205, start to increase in the brain soon after amyloid-beta (Aβ) accumulation, years before neurofibrillary tangles are detected, and decades before symptoms appear. These markers have a larger dynamic range compared to Aβ and can distinguish AD from other forms of dementia more accurately. For instance, phosphorylated tau fragments climb up to 10-fold in blood levels as AD progresses, while the Aβ42/40 ratio falls by only about 15 percent. Monoclonal antibodies such as AT8, AT100, AT180, and AT270 have been developed to target various forms of tau protein. AT8 specifically recognizes tau phosphorylated at serine 202 and threonine 205 sites and has been used to identify abnormal tau protein accumulation in Alzheimer's disease. AT100 targets phosphorylated tau at threonine 212 and serine 214 sites, and has been shown to detect early stage neurofibrillary pathology in AD. AT180 binds to phosphorylated tau at threonine 231 site and is useful in understanding the relationship between tau protein phosphorylation and neurodegeneration. AT270 targets phosphorylated tau at threonine 181 site and has been reported to detect elevated tau protein levels in cerebrospinal fluid of AD patients. These monoclonal antibodies serve as valuable research tools for studying the pathological mechanisms underlying neurodegenerative diseases such as Alzheimer's disease.
